Colonial Pipeline

What happened

Ransomware entered through an unused VPN account with no MFA. Pipeline operations halted. The company paid about USD 4.4 million and still restored from backups because the decryptor was slow.

The door

A leftover remote account, no second factor. Once inside, IT ransomware was enough to force an OT shutdown.

How this stack closes it

Mandatory MFA/SSO on every remote path; unused accounts disabled. Immutable or offline backups so you restore instead of paying. Network segmentation so an office compromise does not halt the plant. Encryption at rest would not have stopped this entry — identity would.

What happened

UnitedHealth CEO Andrew Witty told Congress the attackers used a Citrix remote-access portal without MFA.

Cloud identity with MFA on every portal — no leftover on-prem remote desktop without a second factor. Session logging and least-privilege roles. Kill unused remote-access appliances rather than leave them on the internet.

MOVEit Transfer

What happened

A vulnerability in a public file-transfer product. Data was stolen from thousands of organisations. Backups do not help once a copy has left.

The door

An unpatched public transfer appliance — a SQL-injectable form on the internet, sitting in front of company files.

How this stack closes it

Do not run an unpatched public transfer appliance. Company files live in object storage with IAM, private endpoints, versioning, encryption in transit and at rest, and no SQL-injectable public form. Our Cloud Storage module is built on that model. Encryption at rest does not stop this class of theft; removing the public appliance does.

Equifax

What happened

Unpatched Apache Struts. The patch was issued 7 March 2017 and not applied. Personal data of about 147 million people. Attackers lingered; monitoring was blind in part because of an expired certificate.

The door

A missed patch on a public app, plus an inventory and monitoring failure. A missed patch on a public app is an ops failure — cloud does not magically patch what you hide.

How this stack closes it

Managed patching of internet-facing apps, an inventory of what is on the internet, a WAF, and least privilege so one portal cannot reach the crown-jewel database. Encryption at rest plus audit. What you see on screen depends on the platform that opens the file. Pagination, fonts, table widths, colour rendering, chart behaviour, interactivity and print layout are controlled by the application and version you use — Excel recalculates formulas and lets you filter, PowerPoint paginates to slides, Word paginates to pages, and the HTML report is interactive in the browser. Identical content will therefore look different from one format to another, and can differ between application versions, operating systems and viewers such as Google Docs, Numbers, Keynote, LibreOffice or a mobile preview.
